1. Who we are
blackmenshealthguide.com (the "Site") is owned and operated by PRIME Health Professionals Network, LLC, a Georgia limited liability company ("PHPN," "we," "us," or "our"). "Black Men's Health Guide" is a brand and product line of PHPN.
2. Acceptance of terms
By accessing the Site, signing up for our communications, or purchasing any product from us, you agree to these Terms of Service ("Terms"), our Privacy Policy, and our Medical Disclaimer. If you do not agree, do not use the Site.
3. The products
We offer the following digital products:
- The Black Men's Health Bible, Volume 1 — $29.00 USD, one-time purchase. Delivered as a PDF and DOCX download.
- The Black Men's Health Grid — free PDF download.
- The Black Men's Health Grid Deck — free PPTX download, available after email sign-up.
- The Black Men's Health Grid Matrix — free XLSX download.

4. License granted to you
When you purchase or download any product, PHPN grants you a limited, non-exclusive, non-transferable, revocable license to:
- Download and store one copy of the product for your personal, non-commercial use;
- Print pages for your personal reference; and
- Quote brief excerpts for non-commercial purposes with attribution to "Black Men's Health Guide, PRIME Health Professionals Network, LLC."
You may not:
- Resell, redistribute, sublicense, or share copies of any product;
- Post the product or substantial portions on any website, file-sharing service, or social platform;
- Use the product to train any artificial intelligence or machine learning system;
- Modify, translate, or create derivative works without our written permission; or
- Remove any copyright, trademark, or attribution notices.
All intellectual property rights remain with PHPN.
5. Payment
Payments are processed by Stripe, Inc. ("Stripe"). We do not see or store your full card number. By submitting payment, you agree to Stripe's terms. Prices are in U.S. dollars and may change without notice. The price at the time of your purchase is the price you pay.
6. Refund policy
We offer a 30-day refund on the Black Men's Health Bible. If, within 30 days of your purchase, you decide the product is not for you, email support@blackmenshealthguide.com with your purchase receipt and we will refund your purchase in full. We do not require an explanation. Refunds are processed through Stripe and typically appear on your statement within 5–10 business days.
Free downloads (Grid, Deck, Matrix) are not eligible for refunds because no payment was made.
7. Email communications
If you sign up for any list on the Site, you agree to receive emails from us about the Black Men's Health Guide, including new content, updates, and occasional product announcements. You can unsubscribe at any time using the link in any email or by writing to hello@blackmenshealthguide.com.
8. Educational use only — not medical advice
Everything published on this Site and in our products is general educational content. It is not medical advice and is not intended to diagnose, treat, cure, or prevent any disease. No physician-patient relationship is created between you and William A. Cooper, MD, MBA, or any other contributor to the Guide, by your purchase, download, or reading of any material. 9. Limitation of liability
To the maximum extent permitted by law, PHPN, its members, officers, contributors, and Dr. William A. Cooper are not liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of the Site or any product, including any health outcome, financial loss, or loss of data. Our total aggregate liability to you for any claim arising under these Terms is limited to the amount you paid PHPN in the 12 months preceding the claim, which for free-product users is $0.
10. Indemnification
You agree to indemnify and hold harmless PHPN and its members from any claim, loss, or expense (including reasonable attorneys' fees) arising from your breach of these Terms or your misuse of any product.
11. Intellectual property
All content on the Site and in our products — including text, graphics, logos, the marks "Black Men's Health Guide," "Plan Well. Live Better.," and all related works — is owned by PHPN and protected by U.S. and international copyright and trademark law. Unauthorized use is prohibited.
12. Third-party services
The Site uses third-party services including Stripe (payments), Netlify (hosting and forms), and Google Workspace (email). Your use of those features is also governed by their respective terms.
13. Changes to these Terms
We may update these Terms from time to time. The "Last updated" date at the top will reflect the most recent change. Material changes will be posted on the Site. Your continued use after a change constitutes acceptance.
14. Governing law and disputes
These Terms are governed by the laws of the State of Georgia, without regard to conflict-of-laws principles. Any dispute that cannot be resolved informally will be brought exclusively in the state or federal courts located in Fulton County, Georgia, and you consent to that jurisdiction.
15. Severability
If any provision of these Terms is held unenforceable, the remaining provisions remain in full force and effect.
